Multi-Disciplinary Assessment & Diagnosis for Autism Referrals

Life Connective accepts referrals from parents and relevant professionals for children that they suspect may have an Autism Spectrum Disorder.

Medicare Referrals

Individuals may be able to access Medicare rebates following a GP, specialist or consultant physician referral for the relevant Medicare scheme. 

If eligible, Medicare will rebate a portion of your service fee. The current rebate is $58.00 (as of July 2023).

A Medicare Referral Form with Life Connective’s details must be emailed to us prior to attending your first appointment.

Some relevant Medicare schemes are:

– Chronic Disease GP Management Plans & Team Care Arrangements

– Complex Neurodevelopmental Disorder and Eligible Disability (previously the Better Start for Children with Disability initiative).

– Helping Children with Autism 

– Follow-up allied health services under Medicare for people of Aboriginal and Torres Strait Islander descent

NDIS Referrals

NDIS therapeutic supports referrals can be made directly by the Participant and their families or by other relevant professionals.

Such professionals may be NDIS LACs, Support Coordinators, Psychologists, Occupational Therapists, Physiotherapists and other relevant specialists.

My Aged Care (Home Care) Referrals

Home Care referrals can be made directly by individuals and their families or by various healthcare professionals, including GPs, Geriatricians, Social Workers, Nurses, or other relevant professionals involved in the care of the elderly.

Dysphagia Priority Referrals Eating, Drinking & Swallowing Disorders

Dysphagia referrals are often urgent and thus prioritised.

Dysphagia refers to the difficulty or discomfort in swallowing food, liquids, or saliva. While some cases of dysphagia may be temporary or mild, others can indicate a more serious underlying condition and health outcomes. 

It is important to seek a Speech Pathology appointment promptly.
